The Problem
Dr Rana teaches MRCOG candidates, and her students know her by her method. Moving that teaching online meant more than uploading videos. MRCOG uses question formats that a simple quiz tool cannot model — extended matching questions share one bank of options across several stems — and candidates expect courses, live cohorts and practice in one place, under the teacher's own name. Payments had to suit the candidates she actually teaches: card payments, Fawry and Aman reference codes, and mobile wallets, not only international cards.
- EMQs share one bank of options across several stems — a format generic quiz tools cannot grade.
- Teaching is delivered as courses and live cohorts, not only as practice questions.
- Candidates pay with local methods — Fawry, Aman and wallets — as well as cards.

The Solution
As Dr Rana's technical partner, I designed and developed MRCOG With Rana as her own academy. Its question bank uses a relational model faithful to the source material: options, shared EMQ banks, answer keys, media, corrections and review flags — so single-best-answer, extended-matching and multiple-response questions are all graded correctly on the web platform, with offline grading ready for the mobile apps in development. Around the bank sit a course platform with modules and video, article and resource lessons, free previews and continue-where-you-left-off progress; cohorts with waitlists; and plans that bundle capabilities — the question bank, exclusive exams and selected courses — with courses also sold individually. Website payments run through Fawaterk; the iOS and Android applications, with Apple and Google in-app purchase, are under development.
Approach
- A relational question model: options, shared EMQ banks, answer keys with roles, media, corrections and review flags.
- One grader on the server mirrored in the client, so every format is graded identically everywhere.
- Course → module → lesson platform with video, article and resource lessons and progress tracking.
- Cohorts with waitlists, and plans built as bundles of capabilities.
- A workbook importer behind both the command line and the admin, so content enters through one validated path.
Key Decisions
- Model the question bank on the source, not on the screen: answers stay positional and every new format extends the model additively.
- Fixed-term passes rather than renewing subscriptions — nothing to cancel, nothing to surprise a candidate.
- The DRM capture shield that later protected SmartMRCS was first engineered here.

Architecture Notes
- 1Laravel API and React client; Capacitor shells for iOS and Android are in development.
- 2Fawaterk for website payments (card, Fawry/Aman reference, wallets), with server-verified payment state.
- 3Additive migrations verified on SQLite and MariaDB.

Challenges & Trade-offs
Adding EMQs and multiple-response questions to a platform designed for single-best-answer could have broken every existing reader and report.
A derived projection on each question keeps existing readers fast and compatible, while the relational model underneath holds the real structure.
International payment providers did not fit the way many candidates pay.
Website payments moved to Fawaterk, supporting cards, Fawry and Aman reference codes and wallets, with the server deriving every amount.

Outcomes & Impact
MRCOG With Rana is live at mrcogwithrana.com, carrying the doctor's teaching under her own brand: question bank, courses and cohorts on one account, with payment methods her candidates already use. The iOS and Android applications are under development on the same codebase.
- Live at mrcogwithrana.com under the doctor's own brand.
- Single-best-answer, EMQ and multiple-response questions graded correctly on the web platform.
- Question bank, courses and cohorts on one account, paid with local methods.

Frequently Asked Questions
- Is MRCOG With Rana the same as eMRCOG?
- No. They are separate products for separate clients. MRCOG With Rana is Dr Rana's own academy; eMRCOG is a different MRCOG and MRCPI preparation platform.
- Can you build an academy platform for my own teaching?
- Yes — a question bank in the formats your exam uses, courses and cohorts, the payment methods your learners use, and apps when you are ready. It is your platform, under your name.
